Håvard’s Feywood
In addition to his maps of the Elemental Planes, Håvard also created maps for two other planes: Limbo, and the Feywood (also known as Fairie). Both of these are rather ambiguous, with some considering them to be Inner Planes while others treat them as Outer Planes. Either way, they are intimately connected with Mystara and the Prime Plane.
The main source for the Feywood is of course PC1 Tall Tales of the Wee Folk, although Håvard also brought in other references, as noted on the map itself.
